Most companies will cover the cost of an ocular prosthesis or the care of the prosthesis.
We accept most insurances and will file the claim for the patient.

The insurance company requires that we receive a Letter of Medical Necessity (LMN) from the physician.
We have a LMN form that we can send you.  You can then sign and fax it to our office.
